ParkCity Medical Centre Signs MoU With ManagedCare For Better Patient Outcomes After Discharge

ParkCity Medical Centre (PMC) has entered into a Memorandum of Understanding (MoU) with ManagedCare to provide seamless patient care management and rehabilitation solutions after hospital discharge for better patient outcomes.

The MoU with ManagedCare will see care management and concierge services being made available at PMC to assess post-discharge care requirements including home rehabilitation, home or nursing care, and caregiver training among others.

PMC Acting CEO, Dr Ong Kuo Ghee said “In the aftermath of the pandemic, healthcare providers must continue to evolve to not just better meet changing needs but to enhance value for patients. Aftercare is particularly vital for optimum recovery following complex surgery or specialized treatment. However, there is a gap between post-hospitalization and home care in a patient journey.” 

“We are confident that the multi-disciplinary services provided by ManagedCare will not only ease the transition from hospital to home but also fill a much-needed gap in ensuring proper care is received to mitigate risks post hospitalization. With ManagedCare’s track record, patients can be assured of holistic wellness support and customized support for continuity of care.” 

The collaboration between PMC and ManagedCare will also bridge the gap between acute care in the hospital to short or long-term palliative care at home or in nursing facilities to further improve patients’ quality of life. This serves as an extra layer of support alongside curative treatment and can be provided at any stage of a serious illness.
