Ten Years On, Planetary Health Movement Gains Critical Scientific Alignment

Marking a decade since the Planetary Health field was established, leading scientists have announced a landmark collaboration between the Planetary Health and Planetary Boundaries communities. This alignment is the focus of a new expert commentary in The Lancet, which calls for bold, coordinated action in response to a deepening planetary health emergency. 

Released to coincide with the 10th anniversary of the Rockefeller Foundation–Lancet Commission, the piece underscores that climate change is only one facet of a wider environmental crisis. 

Drawing on insights from both Earth system science and public health, the commentary highlights how accelerating environmental degradation—from biodiversity loss and pollution to land use change and freshwater depletion—is increasingly undermining the ecological foundations of human health and wellbeing.

Moving Beyond Climate Change

The Planetary Boundaries framework, introduced in 2009, identifies nine critical Earth system processes—such as climate stability, biosphere integrity, and chemical pollution—that must remain within defined thresholds to keep the planet in a safe and liveable state. 

These “safe operating spaces” are based on the environmental conditions that have supported human civilisation over the past 10,000 years. However, a 2023 assessment reveals that six of these nine boundaries have already been crossed, significantly raising the risk of destabilising the Earth’s life-support systems.

Mounting evidence shows that this instability poses immediate and serious threats to human health. Environmental disruptions are already affecting air and water quality, food security, the spread of infectious diseases, and the viability of entire communities. 

These impacts contribute to a growing share of the global disease burden, a trend expected to worsen in the years ahead. In response, the Planetary Health field emerged to investigate and address how human-driven environmental change harms health, and to develop solutions that promote ecological balance, equity, and long-term wellbeing.

Uniting Scientific Communities

In response, the Planetary Health field has grown to analyse the links between environmental disruption and human health, while advocating for solutions rooted in justice, sustainability, and resilience. 

“Ten years ago, the Planetary Health community coalesced around the understanding that we cannot have healthy people on an ailing planet,” said co-author Dr Sam Myers. “Today, this collaboration with Planetary Boundaries researchers marks a further maturation of the field—uniting Earth science and public health to address a shared crisis.”

Researchers urge urgent, system-wide changes, outlining four central principles as a guide:

  • Earth system and human health monitoring: Implement detailed, ongoing reviews of how environmental changes affect human health, ensuring policy is guided by the latest findings.
  • Justice-centred policy: Guarantee fair access to essential resources for all, especially those least responsible for environmental harm—including future generations, Indigenous peoples, and marginalised communities.
  • True cost and benefit accounting: Make visible the hidden health costs of environmental degradation, and highlight the tangible benefits of protecting nature. For instance, transforming global food systems could be far less expensive than the long-term consequences of current neglect.
  • Integrated communication: Clearly convey how environmental crises threaten everyone’s health, safety, and livelihoods, while opening pathways for informed, collective action.

Johan Rockström, another co-author, emphasises: “Breaching safe planetary boundaries is already costing lives. We see it in extreme weather, disrupted waterways, pollution, and the connections between ecosystem collapse and pandemics. The health of our planet cannot be separated from human health. Science is now stepping up: not only to highlight these dangers, but to drive the transformation required to protect both our well-being and the stability of societies.”

From Talk to Action

The scientists make clear that individual actions, like recycling and composting, though important, will not be enough. Major systemic changes across food, energy, economic, and policy landscapes are required to confront these unprecedented global challenges. 

By aligning scientific frameworks, policymakers and society at large are offered clearer pathways for putting “Planetary Health in All Policies”, meaning that every policy decision should reflect both public health and planetary stability.

“We’re facing a planetary health emergency that can’t be solved by treating only its symptoms,” says Oskar Masztalerz, another of the piece’s authors. “Justice must be front and centre. Only fundamental systems change, driven by close collaboration across disciplines and societies, will be enough.”

“With the evidence clear and the tools available, the real question is not if major transformation is possible, but whether we’ll respond with the urgency and determination this moment demands,” Dr Myers adds.
